  1. Complete the referral form below
    Give us basic information: the person’s name, contact details, the kinds of services you believe they may benefit from (Host Home, Supported Community Living, etc.), and any relevant notes about their goals or support needs.

  2. We’ll reach out promptly
    Within 1 to 5 business day, a Golden Care coordinator will review the information, clarify any details, and help you understand next steps.

  3. Together, we’ll create a plan
    We’ll schedule an intake conversation, assess eligibility, discuss preferences, and begin crafting a service plan geared toward independence, growth, and community.

  4. We stay connected
    You’re not alone after the referral. Our team will keep you informed of progress, updates, and how you can continue supporting the person’s goals.

To make the referral process smoother, you have to include:

  • The person’s full name, preferred contact method, and best times to reach them

  • Age, current living situation, and support system

  • Diagnoses or medical/behavioral history (as comfortable)

  • Key goals or aspirations (skill building, community involvement, daily living)

  • Any accommodations or preferences (mobility, communication style, schedule)

  • Urgency or special considerations (safety needs, crisis background)
